High Cholesterol Doesn't Hurt. That's What Makes It So Dangerous.

There is no headache. No tightness in the chest. No warning light that flickers on. High cholesterol is completely, thoroughly silent — and that silence is exactly why more than half of adults in England have cholesterol levels above national guidelines, most of them unaware.

Cardiovascular disease — the umbrella term for heart attacks, strokes, and arterial disease caused largely by cholesterol build-up — remains the leading cause of death globally. In the UK alone, 7.6 million people live with heart and circulatory diseases, and one in four premature deaths is attributed to CVD. And behind most of those deaths is a decades-long process that a blood test could have detected — and intervention could have slowed.

The good news? Cholesterol is one of the most modifiable cardiovascular risk factors we have. Diet, exercise, targeted supplementation, and medication where needed can meaningfully reduce it. Modelling shows that if treatment rates for high cholesterol rose from 86% to just 90%, nearly 14,000 heart attacks, strokes, and deaths could be prevented in just three years. But none of that is possible without testing first.

High cholesterol doesn't hurt. It doesn't announce itself. It just quietly builds in your arteries until one day, it causes the most dramatic event of your life.


It's Not All Bad. Here's What's Actually Going On.

Cholesterol gets a bad reputation — but it's an essential molecule. Your body needs it to build cell membranes, produce vitamin D, synthesise hormones (including testosterone and oestrogen), and create bile for digestion. The problem isn't cholesterol itself. It's too much of the wrong type circulating in your blood for too long.

Cholesterol doesn't travel through the bloodstream on its own. It's packaged into carriers called lipoproteins — and it's the type and behaviour of those carriers that determines your cardiovascular risk.

The atherosclerosis process takes decades — and is silent until the crisis stage.

This process — atherosclerosis — takes place over decades. The build-up begins with LDL particles penetrating the artery wall. They oxidise, trigger inflammation, and form the core of a plaque. Over time the plaque grows, hardens, or becomes unstable. When an unstable plaque ruptures and a clot forms, the result is a heart attack or stroke. The first symptom is often the crisis itself.


A Standard Cholesterol Test Is Just the Beginning

A full lipid profile goes well beyond a single "cholesterol number." Here are the key markers — what they measure, and what they mean for your heart health.

The "Bad" — Primary Risk Driver
LDL Cholesterol
Low-density lipoprotein carries cholesterol to tissues — but excess LDL ends up in artery walls. It's not always harmful; it's the small, dense form of LDL that poses the greatest risk, as these particles are more likely to penetrate artery walls and trigger inflammation. For primary prevention, target LDL below 3.0 mmol/L. The European Atherosclerosis Society now says lower is always better — there is no lower limit for LDL reduction.

The "Good" — Protective
HDL Cholesterol
High-density lipoprotein carries cholesterol back to the liver for processing and excretion. Higher HDL is protective. If HDL is less than 1.0 mmol/L, there is increased risk of heart disease independent of other risk factors. Aiming for HDL above 1.2 mmol/L for women and 1.0 mmol/L for men is considered protective. Exercise is the most effective natural way to raise HDL.
Target: >1.0 mmol/L (men), >1.2 (women)
Blood Fats — Diet & Lifestyle
Triglycerides
Triglycerides are fats in the blood, primarily driven by diet — particularly sugar, refined carbohydrates, and alcohol. Elevated triglycerides independently increase cardiovascular risk, and they interact with LDL to create the dangerous small, dense LDL particles. Triglycerides peak at ages 50–59 (1.31 mmol/L overall), with men hitting 1.43 mmol/L — a 32% jump from young adulthood.
Target: <1.7 mmol/L

Total Cholesterol : HDL Ratio
This ratio divides your total cholesterol by your HDL — giving a relative measure of risk that's more meaningful than total cholesterol alone. A high ratio means a lot of potentially dangerous cholesterol relative to the protective HDL clearing it. NICE uses this ratio in its 10-year cardiovascular risk calculation (QRISK3). Aim for below 4.0, ideally below 3.5.

Advanced Marker — Particle Count
ApoB (Apolipoprotein B)
Small apolipoprotein B-containing lipoproteins can cross the vascular endothelial barrier and accumulate in the arterial wall, leading to atheromatous plaque formation. ApoB measures the total number of atherogenic (plaque-forming) particles in your blood — not just how much cholesterol they carry. It can reveal hidden cardiovascular risk in people whose LDL looks normal. Increasingly recommended by cardiologists as the most accurate single marker of heart risk.

Genetic Risk — Often Missed
Lp(a) — Lipoprotein(a)
Lp(a) is a genetically determined form of LDL that significantly increases cardiovascular risk — and is completely unaffected by diet or lifestyle. Around 1 in 5 people carry elevated Lp(a), and most have no idea. It was recently added to NICE's guidelines on lipids. It should be tested once in a lifetime — if elevated, it changes how aggressively other risk factors need to be managed.

Why "normal" total cholesterol can still mean high risk

A total cholesterol level that is high could be due to elevated levels of beneficial HDL cholesterol, thus not necessarily indicating an increased risk of heart disease. To understand your heart health, explore the key cholesterol types, triglycerides and their ratios. Equally, someone with "normal" total cholesterol but low HDL, high triglycerides, and elevated small dense LDL particles can have significant cardiovascular risk that a single number completely misses. This is why a full lipid profile — not just a total cholesterol figure — is so important.

The menopause cholesterol shift

Oestrogen has a protective effect on cholesterol — it helps keep LDL lower and HDL higher. When oestrogen declines at menopause, women's lipid profiles often change significantly and rapidly. This is why NHS England specifically identifies women who have been through the menopause as a high-risk group requiring priority cholesterol screening. Many women find their cholesterol rises noticeably within 1–2 years of menopause — making testing in the perimenopausal period particularly important.


How to Improve Your Cholesterol — Starting Now

Here's the empowering part: cholesterol is one of the most responsive cardiovascular risk factors to lifestyle intervention. For most people, meaningful improvements are achievable within 8–12 weeks of consistent changes.

Get a full lipid profile — not just total cholesterol

Ask for LDL, HDL, triglycerides, non-HDL, and the TC:HDL ratio as a minimum. If you want the complete cardiovascular picture, add ApoB and — at least once in your life — Lp(a). Private blood testing makes this straightforward and fast, with results in 1–3 days.

Overhaul your fat quality, not just quantity

Replacing saturated fats (found in processed meats, full-fat dairy, baked goods) with unsaturated fats (oily fish, avocado, olive oil, nuts) consistently reduces LDL. The Mediterranean diet pattern has the strongest evidence base for cardiovascular risk reduction of any dietary approach.

Cut sugar and refined carbs — they drive triglycerides

Elevated triglycerides are primarily a dietary problem — specifically excess sugar, refined carbohydrates, and alcohol. Reducing these directly lowers triglycerides and improves the ratio of large, fluffy LDL (less harmful) to small, dense LDL (more harmful). This is one of the fastest-responding lipid markers to dietary change.

Exercise — especially for HDL

Regular aerobic exercise is the most effective natural way to raise HDL cholesterol. Even 150 minutes of moderate activity per week produces meaningful improvements. Resistance training also has benefits — reducing LDL and improving insulin sensitivity, which in turn improves triglycerides.

Consider plant sterols and omega-3s

Plant sterols (found in fortified foods and supplements) have clinical evidence for reducing LDL by 7–10% when taken consistently at 2g per day. Omega-3 fatty acids (fish oil) are particularly effective for lowering elevated triglycerides. These aren't replacements for lifestyle changes — but they're genuinely effective additions.

Discuss statins and other treatments openly with your GP

Statins are among the most well-evidenced medications in the history of medicine — safe, effective, and widely available. For those with elevated LDL or high calculated cardiovascular risk, they dramatically reduce heart attack and stroke risk. If your GP has recommended them, take that seriously. If you have concerns, discuss them — don't just avoid the conversation.

Cholesterol is modifiable. Heart disease is preventable. But only if you know your numbers — and only if you act on them early enough to make a difference.
