The results are remarkable. Clinical trials show average weight loss of 10–15% of body weight with semaglutide and 15–22% with tirzepatide. Real-world UK data from 113,630 people through one private provider showed average weight loss of 21.5% over 10 months. The result: an estimated 2.4 million people are currently accessing GLP-1 medications privately in the UK.

The contraindications — who cannot take GLP-1 medications
GLP-1 medications are not appropriate for everyone. People with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2 should not take them. They are not recommended during pregnancy or breastfeeding. People with a history of pancreatitis, severe gastrointestinal disease, or certain kidney conditions require careful clinical assessment. Before starting any GLP-1 medication — NHS or private — insist on a baseline blood panel. At minimum: HbA1c, full lipid profile, liver function, kidney function, full blood count, and thyroid. A good private clinic will require this. If yours doesn't, that's a red flag about their clinical governance.
Understand and address your metabolic root causes
If your blood tests reveal insulin resistance, thyroid dysfunction, high cortisol, or hormonal imbalance — these deserve specific attention alongside GLP-1 treatment, not just the medication. Addressing root causes makes treatment more effective and reduces the risk of weight regain when medication stops.
Prioritise protein and resistance training from day one
Aim for 1.6–2.0g of protein per kg of target body weight daily. Combine with resistance training at least 3x per week. This combination is the most evidence-based approach to preserving muscle mass during rapid weight loss on GLP-1 medications.
